ABSTRACT BACKGROUND: Although autonomic dysfunction has been shown to be associated with liver cirrhosis, the prevalence and prognostic implications are unclear. Abnormal heart rate variability (HRV), a measure of autonomic function, has not been well investigated in cirrhosis. OBJECTIVE: To evaluate the prevalence of high-risk HRV parameters in a cohort of cirrhotic patients and their association with cardiac dysfunction and mortality. DESIGN AND SETTING: Prospective observational study conducted in the Federal University of São Paulo. METHOD: A cohort of 120 patients, comprising 17 healthy controls and 103 cirrhotic outpatients, was evaluated and followed for 10 months. HRV analysis was based on 24-hour Holter monitoring and defined using time-domain and frequency-domain parameters. RESULTS: The HRV parameters were statistically lower in cirrhotic patients than in healthy subjects. High-risk HRV parameters were prevalent, such that 64% had at least one high-risk parameter. Time-domain parameters correlated with Child scores (P < 0.0001). In regression models, HRV parameters were independent predictors of diastolic dysfunction and mortality. During 10 months of follow-up, there were 11 deaths, all of patients with at least one high-risk HRV parameter. Kaplan-Meier analysis estimated low survival rates among patients with standard deviation of normal-to-normal RR intervals (SDNN) < 100. CONCLUSION: Reduced HRV is prevalent in liver cirrhosis and is related to cardiac dysfunction, severity of liver disease and mortality. Abnormal high-risk HRV parameters are prevalent among cirrhotic patients and are also predictors of mortality. Our findings highlight the need for a more careful cardiac evaluation of cirrhotic patients.

Abstract Objective To evaluate objective criteria of Magnetic Resonance Imaging (MRI) of Placenta Accreta Spectrum disorder (PAS) analyzing interobserver agreement and to derive a model including imaging and clinical variables to predict PAS. Methods A retrospective review including patients submitted to MRI with suspicious findings of PAS on ultrasound. Exclusion criteria were lack of pathology or surgical information and missing or poor-quality MRI. Two radiologists analyzed six MRI features, and significant clinical data were also recorded. PAS confirmed on pathology or during intraoperative findings were considered positive for the primary outcome. Variables were tested through logistic regression models. Results Final study included 96 patients with a mean age of 33 years and 73.0% of previous C-sections. All MRI features were significantly associated with PAS for both readers. After logistic regression fit, including MRI signs with a moderate or higher interobserver agreement, intraplacental T2 dark band was the most significant radiologic criteria, and ROC analysis resulted in an AUC = 0.782. After including the most relevant clinical data (previous C-section) to the model, the ROC analysis improved to an AUC = 0.893. Conclusion Simplified objective criteria on MRI, including intraplacental T2 dark band associated with clinical information of previous C-sections, had the highest accuracy and was used for a predictive model of PAS.

RESUMO A lesão renal aguda é uma complicação comum da cirrose, acometendo até 20% dos pacientes hospitalizados. Este tema está em rápida evolução devido aos importantes avanços em novas classificações, biomarcadores e tratamentos que ocorreram nos últimos anos. Em nome da Sociedade Brasileira de Hepatologia, um painel de especialistas em Hepatologia e Nefrologia revisou as evidência publicadas na literatura, integrando os diferentes resultados de estudos, para desenvolver a presente recomendação.

ABSTRACT Acute kidney injury is a common complication of cirrhosis, occurring in up to 20% of patients hospitalized with cirrhosis. This field is rapidly changing, with significant advances in classification, biomarkers and therapy over the last few years. On the behalf of the Brazilian Society of Hepatology, a panel of experts in Hepatology and Nephrology reviewed published evidence to integrate findings and develop the recommendations presented in this manuscript.

BACKGROUND: Spinal anesthesia for cesarean delivery may cause severe maternal hypotension and a decrease in cardiac output. Compared to assessment of cardiac output via a pulmonary artery catheter, the FloTrac/Vigileo™ system may offer a less invasive technique. The aim of this study was to evaluate cardiac output and other hemodynamic measurements made using the FloTrac/Vigileo™ system in patients undergoing spinal anesthesia for elective cesarean section. METHODS: A prospective study enrolling 10 healthy pregnant women was performed. Hemodynamic parameters were continuously obtained at 15 main points: admission to surgery (two baseline measurements), after preload, after spinal anesthesia administration and 4 time points thereafter (4, 6, 8 and 10 min after anesthesia), at skin and uterine incision, newborn and placental delivery, oxytocin administration, end of surgery, and recovery from anesthesia. Hemodynamic therapy was guided by mean arterial pressure, and vasopressors were used as appropriate to maintain baseline values. A repeated measures ANOVA was used for data analysis. RESULTS: There was a significant increase in heart rate and a decrease of stroke volume and stroke volume index up to 10 min after spinal anesthesia (P < 0.01). Importantly, stroke volume variation increased immediately after newborn delivery (P < 0.001) and returned to basal values at the end of surgery. Further hemodynamic parameters showed no significant changes over time. DISCUSSION AND CONCLUSIONS: No significant hemodynamic effects, except for heart rate and stroke volume changes, were observed in pregnant women managed with preload and vasopressors when undergoing elective cesarean section and spinal anesthesia.

JUSTIFICATIVA E OBJETIVOS: Adição de clonidina subaracnóidea (±-agonista) prolonga a ação analgésica da combinação sufentanil e bupivacaína isobárica em analgesia combinada para o trabalho de parto ¹. O objetivo deste estudo foi comparar a qualidade de analgesia e a prevalência de efeitos colaterais após a adição de clonidina subaracnóidea à solução anestésica em gestantes durante trabalho de parto. MÉTODO: Após aprovação da Comissão de Ética, 22 gestantes em trabalho de parto receberam aleatoriamente no espaço subaracnóideo 2,5 mg de bupivacaína hiperbárica 0,5% (grupo CLON/HIPER; n = 11) ou 2,5 mg de bupivacaína isobárica 0,5% (grupo CLON/ISO; n = 11) em associação ao sufentanil 2,5 µg e à clonidina 30 µg. A dor avaliada pela Escala Analógica Visual, a freqüência cardíaca e a pressão arterial média foram estudadas a cada 5 minutos nos primeiros 15 minutos e, a seguir, a cada 15 minutos até o nascimento. Foi avaliada a prevalência de efeitos colaterais (náusea, vômito, prurido e sedação). O estudo foi encerrado no momento em que foi necessária complementação analgésica peridural (dor > 3 cm) ou ao nascimento. A análise estatística foi realizada pelos testes t de Student, Qui-quadrado, Fisher e ANOVA de duas vias para medidas repetidas, considerando como significativo p < 0,05. RESULTADOS: Os grupos CLON/HIPER e CLON/ISO foram semelhantes com relação a dados antropométricos, duração da analgesia (70,9 ± 32,9 vs. 85,4 ± 39,5), freqüência cardíaca, ocorrência de prurido, sedação, náusea e vômitos. No grupo CLON/ISO houve diminuição significativa da pressão arterial média com relação ao grupo CLON/HIPER nos momentos 15, 30 e 45 minutos (p < 0,05). CONCLUSÕES: Nas condições estudadas, a adição de clonidina em baixa dose (30 ¼g), associada ao sufentanil, determinou maior ocorrência de hipotensão quando administrada com soluções isobáricas de anestésico local. Com relação aos demais efeitos colaterais, as soluções hiperbáricas e isobáricas comportam-se de maneira semelhante.

BACKGROUND AND OBJECTIVES: The addition of subarachnoid clonidine (α-agonist) prolongs the analgesia produced by the combination of sufentanil and isobaric bupivacaine in combined labor analgesia¹. The objective of this study was to compare the quality of analgesia and the prevalence of side effects after the addition of subarachnoid clonidine to the anesthetic solution in labor analgesia. METHODS: After approval by the Ethics Commission, 22 pregnant women in labor were randomly assigned to the subarachnoid administration of either 2.5 mg of 0.5% hyperbaric bupivacaine (CLON/HYPER Group; n = 11) or 2.5 mg of 0.5% isobaric bupivacaine (CLON/ISO Group; n = 11) associated with 2.5 µg of sufentanil and 30 µg of clonidine. Pain, evaluated by the Visual Analogue Scale, heart rate, and mean arterial pressure were assessed every 5 minutes during the first 15 minutes, and then every 15 minutes afterwards until delivery. The prevalence of side effects (nausea, vomiting, pruritus, and sedation) was evaluated. The study was terminated whenever the patient needed supplemental epidural analgesia (pain > 3) or upon delivery of the fetus. The Student t test, Chi-square test, Fisher exact test, and two-way ANOVA for repeated measurements were used in the statistical analysis and a p < 0.05 was considered significant. RESULTS: Anthropometric data, duration of analgesia (70.9 ± 32.9 vs. 85.4 ± 39.5), heart rate, and the incidence of pruritus, sedation, nausea, and vomiting were similar in both groups. Mean arterial pressure was significantly lower in the CLON/ISO Group than in the CLON/HYPER Group at 15, 30, and 45 minutes (p < 0.05). CONCLUSIONS: Under the conditions of the present study, the association of a small dose of clonidine (30 µg) with sufentanil caused a higher incidence of hypotension when the isobaric solution of the local anesthetic was used. For all other side effects, both hyperbaric and isobaric solutions showed similar behavior.

JUSTIFICATIVA Y OBJETIVOS: La adición de la clonidina subaracnoidea (±-agonista), prolonga la acción analgésica de la combinación sufentanil y bupivacaína isobárica en analgesia combinada para el trabajo de parto ¹. El objetivo de este estudio fue comparar la calidad de analgesia y la prevalencia de los efectos colaterales, después de la adición de clonidina subaracnoidea a la solución anestésica en gestantes durante el parto. MÉTODO: Después de la aprobación de la Comisión de Ética, 22 gestantes en trabajo de parto recibieron aleatoriamente en el espacio subaracnoideo 2,5 mg de bupivacaína hiperbárica 0,5% (grupo CLON/HIPER; n = 11) o 2,5 mg de bupivacaína isobárica 0,5% (grupo CLON/ISO; n = 11) en asociación con el sufentanil 2,5 µg y la clonidina 30 µg. El dolor evaluado por la Escala Analógica Visual, la frecuencia cardíaca y la presión arterial promedio, fueron estudiados a cada 5 minutos en los primeros 15 minutos y a continuación, a cada 15 minutos hasta el nacimiento. Fue evaluada la prevalencia de efectos colaterales (náusea, vómito, prurito y sedación). El estudio fue terminado en el momento en que se hizo necesaria la complementación analgésica epidural (dolor > 3 cm) o al nacimiento. El análisis estadístico fue realizado a través de los tests t de Student, Chi-cuadrado, Fisher y ANOVA de dos vías para medidas repetidas, considerando como significativo p < 0,05. RESULTADOS: Los grupos CLON/HIPER y CLON/ISO fueron similares con relación a los datos antropométricos, duración de la analgesia (70,9 ± 32,9 vs. 85,4 ± 39,5), frecuencia cardíaca, incidencia de prurito, sedación, náusea y vómitos. En el grupo CLON/ISO hubo una disminución significativa de la presión arterial promedio con relación al grupo CLON/HIPER en los momentos 15, 30, y 45 minutos (p < 0,05). In order to evaluate the accuracy of a urine reagent dipstick (Multistix 10SG®) to determine ascitic fluid leukocyte count, we prospectively studied 106 cirrhotic patients from April 2003 to December 2004, in two different centers (Federal University of São Paulo - UNIFESP-EPM and Federal University of Juiz de Fora - HU-UFJF) for the rapid bedside diagnosis of spontaneous bacterial peritonitis. The mean age 54 ± 12 years, there was a predominance of males (eighty-two patients, 77%), and alcohol was the most frequent etiology (43%). Forty-four percent of patients were classified as Child B and fifty-one as Child C (51%). Abdominal paracentesis was performed both in outpatient and inpatient settings and the Multistix 10SG® was tested. Eleven cases of spontaneous bacterial peritonitis were identified by means of polymorphonuclear count. If we considered the positive Multistix 10SG® result of 3 or more, the sensitivity, specificity, positive and negative predictive value were respectively 71%, 99%, 91% and 98%. With a positive reagent strip result taken as grade 2 (traces) or more, sensitivity was 86% and specificity was 96% with positive and negative predictive values of 60% and 99%, respectively. Diagnostic accuracy was 95%. We concluded that the use of a urine reagent dipstick (Multistix 10SG®) could be considered a quick, easy and cheap method for ascitic fluid cellularity determination in SBP diagnosis.

RACIONAL: Existe associação entre doença óssea metabólica e doença hepática colestática. Contudo, a associação com cirrose não-colestática ainda é pouco conhecida. OBJETIVOS: Determinar a prevalência e a gravidade da perda de densidade mineral óssea na cirrose não-colestática e investigar fatores preditivos do seu diagnóstico. MÉTODOS: Oitenta e nove pacientes e 20 controles foram estudados de março a setembro de 1998. Todos foram submetidos a exames laboratoriais e densitometria óssea da coluna lombar e do colo do fêmur. RESULTADOS: A massa óssea estava significativamente reduzida em ambos os sítios nos pacientes quando comparado aos controles. A prevalência da perda de massa óssea na cirrose não-colestática, de acordo com os critérios da Organização Mundial da Saúde, foi de 78% na coluna lombar e 71% no colo do fêmur. A massa óssea diminuiu significativamente com a idade em ambos os sítios, especialmente em pacientes acima de 50 anos. Pacientes mulheres pós-menopausa tinham massa óssea significativamente menor do que pacientes mulheres pré-menopausa e homens em ambos os sítios. Não houve diferença significativa na massa óssea entre as etiologias não-colestáticas. A massa óssea da coluna lombar diminuiu significativamente com a progressão da disfunção hepática. Nenhuma variável bioquímica foi associada com a perda da massa óssea. CONCLUSÕES: A perda de massa óssea foi freqüente em pacientes com cirrose não-colestática. Pacientes idosos, do sexo feminino na pós-menopausa e com disfunção hepática grave apresentaram doença óssea mais avançada. Os exames laboratoriais rotineiramente dosados nos pacientes com doença hepática não puderam predizer com segurança a presença de redução na massa óssea.

BACKGROUND: Metabolic bone disease has long been associated with cholestatic disorders. However, data in noncholestatic cirrhosis are relatively scant. AIMS: To determine prevalence and severity of low bone mineral density in noncholestatic cirrhosis and to investigate whether age, gender, etiology, severity of underlying liver disease, and/or laboratory tests are predictive of the diagnosis. PATIENTS/METHODS: Between March and September/1998, 89 patients with noncholestatic cirrhosis and 20 healthy controls were enrolled in a cross-sectional study. All subjects underwent standard laboratory tests and bone densitometry at lumbar spine and femoral neck by dual X-ray absorptiometry. RESULTS: Bone mass was significantly reduced at both sites in patients compared to controls. The prevalence of low bone mineral density in noncholestatic cirrhosis, defined by the World Health Organization criteria, was 78% at lumbar spine and 71% at femoral neck. Bone density significantly decreased with age at both sites, especially in patients older than 50 years. Bone density was significantly lower in post-menopausal women patients compared to pre-menopausal and men at both sites. There was no significant difference in bone mineral density among noncholestatic etiologies. Lumbar spine bone density significantly decreased with the progression of liver dysfunction. No biochemical variable was significantly associated with low bone mineral density. CONCLUSIONS: Low bone mineral density is highly prevalent in patients with noncholestatic cirrhosis. Older patients, post-menopausal women and patients with severe hepatic dysfunction experienced more advanced bone disease. The laboratory tests routinely determined in patients with liver disease did not reliably predict low bone mineral density.
